2004 Gathering Workshop Registration Form

Please indicate  4 workshop choices for Saturday Half-Day, Full Day or One and a Half-Day Workshops and 4 choices for Sunday Half-Day Workshops. Please use the Workshop Number as shown on the Workshop List to indicate your choices. Workshop assignment is done in registration number order.  To ensure fairness, all registration forms received by October 15, 2003 will be pulled randomly and numbered sequentially. Registrations arriving after October 15th will be assigned the next available number(s) on the day they are received.

Please include payment for your number 1 workshop choice for both Saturday and Sunday.  If you are assigned to an alternate workshop, you will be advised as to how we will handle any difference in price.
